10/31/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a content mood evident by her facial expressions such as a smile on the face and engaging in general conversation with QMHPA. Intervention: QMHPA inquired about Ms. Broadnax’s current thoughts, feelings, and emotions. QMHPA encouraged the use of the deep breathing exercises. QMHPA used modeling to teach Ms. Broadnax how to take three deep breaths, relax and in-hale. QMHPA encouraged Ms. Broadnax to concentrate on peaceful thoughts during the exercises. QMHPA also educated Ms. Broadnax on the symptoms of Bi-polar disorder/ Depressive Disorder. QMHPA reinforced the importance of gaining knowledge through connecting with a therapist to help in reducing stress and stressful situations. QMHPA probed Ms. Broadnax regarding the impact of her inability to sleep has played on her start of the day. QMHPA suggests identifying triggers that has caused her to be “stressed”. Response: Ms. Broadnax appeared to be receptive to QMHPA’s services and interventions that were provided to her during the session. Ms. Broadnax shared that she did not sleep good last night because her neighbors were having a party and they were up until 5 this morning. Ms. Broadnax shared she had a bad headache and needed to get out and get some fresh air. Ms. Broadnax was able to process through weighing the pros and cons that she must be conscious of her actions because she does work, and she does need her job even though she does not like the new management and especially after she has learned that her daddy is working there. Ms. Broadnax that she must set limits for herself as well as set boundaries and be more assertive in her decision making. Ms. Broadnax also acknowledged that when communicating her needs she must learn to avoid stressful situations that may trigger her negative behaviors. Ms. Broadnax admits that when she does not have her proper rest she will become easily annoyed by certain situations. Ms. Broadnax indicated to QMHPA she has been adhering to her medication management on a consistent basis and now she is just worried about the other stuff she has to deal with for a lifetime. Plan: QMHPA will continue to teach Ms. Broadnax how to identify as well as process triggers and educate her regarding positive coping skills and interpersonal skills 1-2 times (Weekly).
